Psychological Testing Informed Consent
Psychological assessment allows for the ability to obtain information about a person's psychosocial, interpersonal, and cognitive well-being and provide the client, parent/guardian, etc., with relevant clinical findings and beneficial recommendations. If you have been referred to testing, please read through and sign the following form ahead of your first appointment.

We bill insurance for our services and have a sliding fee option for those who qualify. If you have a question about your bill, we're happy to help.
