Case Management
Having a child with severe and long-term mental health needs can be overwhelming, confusing, tiring, and expensive. To help, our case management team reduces stress and provides clarity without adding cost.
What is Case Management, and who is it for?
Our case management staff helps families manage the natural difficulties of having a child with mental health issues that affect their ability to function at home, in school, or in the community. We help families find and access resources provide connections to services and make sure services stay effective as the child grows and changes. Families may have multiple children receiving case management services at the same time and a clinician will call you to discuss options if more than two siblings qualify for services concurrently.
How often do families see case managers, and where?
A face-to-face meeting with the child and/or child’s parent or guardian is required at least once every month. It may be necessary and beneficial to meet more frequently, based on the level of need. Meetings can take place in the child’s home, school, or in the community.
Who is eligible?
Children ages 3 to 17 who live in Hennepin County are eligible. They must have a qualifying mental health diagnosis and experience significant impairment at home, school, or in the community. The child must be assessed by a mental health professional to determine a qualifying diagnosis and level of functioning.
What does it cost?
Case Management Services are provided at no cost to families, regardless of income. Services are covered by Hennepin County funding or insurance, with no co-pays or any other fees.
What consent is necessary for services?
Because our services fall under Hennepin County funding or insurance, we need the consent of a parent or guardian.
Is a referral needed?
Referrals can be made by mental health professionals or the child’s school, but parents and guardians can also contact us directly.
